HANOVER - On Saturday at about 5 p.m. Chautauqua County Sheriff's deputies responded to a collision between a semi and a car on Route 39/Hanover Road.
Deputies reported that David Adams, 63, of Hanover Road in Forestville, who was operating a semi without a trailer attached, entered the intersection and failed to yield to a vehicle operated by Sandra Cassidy of Bliss. Cassidy was traveling east on Route 39.
Adams collided with Cassidy's front driver's side. Sandra Cassidy and her passenger, Katie Cassidy, were taken to Lake Shore Hospital by Forestville EMS for minor injuries.
Adams was issued a uniform traffic ticket for failure to yield right of way. He is scheduled to appear in the Town of Hanover Court at a later date.
State Police at Jamestown
SINCLAIRVILLE - On Wednesday, Roberta Price, 50, of Jamestown was charged with fourth-degree criminal mischief and second-degree harassment in connection to a report of a domestic dispute at the Sinclairville Firehall.
Reportedly Price punched the victim several times, breaking her glasses in the process.
Price was processed at state police barracks in Jamestown and issued appearance tickets returnable to the Town of Char-lotte Court at a later date.
LAKEWOOD - On Friday, Frank Salapek, 57, of Jamestown was charged with felony driving while intoxicated.
A trooper pulled over Salapek, who was operating a motorcycle on Fairmount Avenue, for driving erratically. Salapek was transported to state police headquarters in Jamestown where he refused to take a breath test.
A check of his driving record reportedly showed two DWI convictions within the past 10 years, making the charge a felony.
He was processed, arraigned in the Town of Ellicott Court, and remanded to the Chautauqua County Jail.
On Saturday, Clifford Cummings, 54, of Salamanca was charged with driving while intoxicated while his passengers, Michael Redeye, 28, of Salamanca and Leon Kindt, 45, of Jamestown, were charged with unlawful possession of marijuana.
Troopers stopped a vehicle driven by Cummings on Arterial Street in Jamestown for a traffic violation. Cummings backed into the state police vehicle causing minimal damage. Cummings reportedly failed several field sobriety tests.
It was reported that Redeye and Kindt were in possession of bags containing marijuana. They were both issued appearance tickets for the City of Jamestown Court and released.
Cummings was transported to state police barracks in Jamestown where he provided a breath sample that was reported to be .14 percent.
He was processed, issued tickets returnable to the City of Jamestown Court and released to a sober individual.
ELLICOTT - On Saturday, Isabel Rodas-Churimia, 50, of Dunkirk was charged with driving while intoxicated.
State police were notified of a vehicle all over the road on Route 60. A trooper located the vehicle and stopped it. Reportedly, Rodas-Churimia failed several field sobriety tests.
He was transported to state police barracks in Jamestown where his breath sample was reported to be .13 percent. He was processed, arraigned in the Town of Ellicott Court and remanded to the Chautauqua County Jail.
