Home care options available

If you need help at home, there are a few options available. The type of care is based on what you need and for how long. There are short-term needs that require a skilled service like a nurse or physical therapist.

This may be a return to your home after a procedure like a hip or knee replacement. Typically, your health insurance covers this short-term service.

At NY Connects, our staff is available to you to learn and be connected to community-based support and information on services, programs, and opportunities. Every person and situation is different. If you need care and have questions, you can call NY Connects for assistance and information on these topics.

The Chautauqua County Office for Aging Services has a non-medical home care program for older adults over 60 years of age who need assistance in their homes to remain independent. CASA assists clients who are on Medicaid and need medical support services in the home. Managed Long Term Care (MLTC) providers also provide Medicaid clients in the community with long-term community-based support and services. The Veterans system can provide some assistance for veterans in the community. Some Medicare Advantage plan insurance plans can provide some short-term services and some additional services like short-term home-delivered meals after a hospital or nursing home short stay or a personal emergency response system or help button. Each of these programs has criteria and an application process that may need to be completed.
