Chautauqua Hospice and Palliative Care recently relocated their Northern Chautauqua County office from West Main Street in the village of Fredonia to 3780 Eagle St., Fredonia.

CHPC’s Fredonia office is utilized by its treatment teams, administration and board for charting, training and meeting space. Patients and families needing assistance are encouraged to call CHPC’s Fredonia line at 672-6944 rather than visit the office, as staff are not regularly on premise in this location.

The mission of Chautauqua Hospice & Palliative Care is to provide end-of-life education and care. CHPC has served the residents of Chautauqua County for over 35 years, providing quality care for patients and families facing life-limiting terminal illness, as well as palliative care for those with serious chronic illness.
