Area providers join together to form Chautauqua Medical Practice
WESTFIELD — Fifteen providers — including physicians, a physician assistant and a nurse practitioner — have joined together to form The Chautauqua Medical Practice, with four locations in Western New York.
The providers are all on the medical staff at Westfield Memorial Hospital, part of the Allegheny Health Network.
¯ The Chautauqua Medical Practice includes: Chautauqua Medical Practice – Bemus Point, 4936 Main Street in Bemus Point, and includes family physician Dr. Paul S. Holley and nurse practitioner Holly Reed. Practice phone is 386-2414.
¯ The Chautauqua Medical Practice – Westfield, 158 E. MainSt., and includes family practice physician Dr. Russell Elwell and David Trusel, RPA-C.. The practice phone is 326-4686.
¯ The Chautauqua Medical Practice – Ripley, 1 Goodrich St., includes David Trusel, RPA-C, whose specialty is family medicine. Practice phone is 736-6300.
As part of Allegheny Health Network’s commitment to bringing care closer to home, seven specialty physicians who practice in Erie as part of AHN Saint Vincent Hospital offer services in Westfield and are part of the Chautauqua Medical Practice. They include:
¯ Dr. Vladislava Buntic, a gastroenterologist who sees patients at the Specialty Suite Services on the second floor of Westfield Memorial Hospital at 189 E. Main St. Appointments can be scheduled through Digestive Diseases of Northwestern PA at 814-452-2767.
¯ Dr. Mark Bloomstine, an orthopaedic surgeon who sees patients at the Specialty Suite Services on the second floor of Westfield Memorial Hospital. Appointments can be scheduled through the AHN Saint Vincent Orthopaedic Institute at 814-454-2401.
¯ Dr. Keven Schattner, a general surgeon who sees patients at the Specialty Suite Services on the second floor of Westfield Memorial Hospital. Appointments can be scheduled through Greater Erie Niagara Surgery at 814-454-1142.
¯ Dr. Ross Peterson, Dr. Para Yogeswaran and Dr. Nishant Koradia, cardiologists who provide care for patients at the Specialty Suite Services on the second floor of Westfield Memorial Hospital. Appointments can be made at AHN Saint Vincent Consultants in Cardiology at 814-453-7767.
¯ Dr. Burkhart Zorn, a urologist who sees patients at the Specialty Suite Services office on the second floor of Westfield Memorial Hospital. Appointments can be made at AHN Saint Vincent Allied Urology at 814-452-4214.
In addition, Chautauqua Medical Practice physicians are providing new specialty telehealth programs, including endocrinology and rheumatology.
Endocrinology care is provided by Dr. Kersthine Andre and Dr. Paul Custodio, and rheumatology care is provided by Dr. Christina Payne. The Westfield Memorial Hospital specialty telehealth services are provided utilizing an InTouch telehealth cart located inside the Meyer Cottage at 185 E. Main St., Westfield. The device allows for two-way video and voice communication between the patient and the provider. Online appointments can be made by calling 793-2101
Additional telehealth and in-person specialist care are slated to be added in the coming months.
All Chautauqua Medical Practice physicians are credentialed with New York health insurance plans.
