American Legion Auxiliary Unit 327 is sponsoring a QuestDiagnostics annual blood analysis wellness program tomorrow, Saturday, from 6 to 10 a.m. at Post 327, 7579 E. Main Road, Westfield.

Call 1-800-234-8888 today to make a reservation. Operators are available from 8:30 a.m. to 5 p.m. Callers must have their doctor’s name and address available when calling. Personal physician authorization is required, as well as photo IDs for participants and PHI forms for patient copies.

The AMBA Panel includes Coronary Disease: Cholesterol, HDL Cholesterol, Cholesterol/HDL ratio, Triglycerides, LDL Cholesterol; Diabetes: Glucose; Kidney Disease: Blood Urea Nitrogen, Creatinine, BUN/Creatinine-Ratio GFR); Anemia: Red Blood Indices (MCV, MCHC, RDW), Hemoglobin, Hematocrit, Red Blood Cell Count, White Blood Cell Count; Liver Disease: Albumin, Globulin, Albumin/Globulin Ratio, AST, ALT, Total Billirubin, Total Protein, Alkaline Phosphatase; Other Tests: Calcium, Chloride, Potassium, Sodium, Carbon Dioxide. The cost for the AMBA Panel, which includes all the tests listed above, is $41. Note: this screening is not covered by Medicare and most other insurance companies. Check or money order only will be accepted the day of the program and should be made payable to AMBA Wellness Programs. The sponsor group may charge a separate handling fee.

Optional tests include 5363 Prostate Specific Antigen (PSA) $25.70; 899 TSH $12; 17306 Vitamin D-25 $25.50; 496 Hemoglobin A1C $12; 11290 Insure Colon Rectal Kit $21. A separate physician order is required for any of the optional tests. Present a separate physician order at the time of the testing if electing an optional test. There will be no exceptions at the program.
